Staying on Track: Local Evaluation
The Initial Local Evaluation Report Annual Summary and Planning Report is open. Please use the link to complete the Microsoft form by October 31st. You will need your Local Evaluation Initial Local Evaluation Feedback Report (emailed in January 2026) and your Related Performance Indicators Report (Interim sent on 9/4; final anticipated by 9/30) for reference. A PDF version of the report is available in the Resource Center for review. Those files can also be accessed in the tabbed section below. You must complete 1 report for each grant cohort you have.

Initial Local Evaluation Annual Summary and Planning Report – October 2026
This report template is for both 21st Century Community Learning Centers (CCLC) and Lottery for Education Afterschool Programs (LEAPs) Grants. If an agency has both 21st CCLC and LEAPs funding, then complete separate reports for each grant. If an agency has more than one 21st CCLC Grant, separate reports should be completed for each cohort. The deadline to submit the report is October 31, 2026.
To complete this report, you will need your 2025-26 Performance Indicators Report and either a copy of the Initial Local Evaluation Report you submitted last year or the
Initial Local Evaluation Feedback sent to you from UT SWORPS. If you are a new grantee, you may use the state performance goals as your program goals.
Mid-Year Local Evaluation Report – January 2027
This report template is for both 21st Century Community Learning Centers (CCLC) and Lottery for Education Afterschool Programs (LEAPs) Grants. If an agency has both 21st CCLC and LEAPs funding, then complete separate reports for each grant. If an agency has more than one 21st CCLC Grant, separate reports should be completed for each cohort. The deadline to submit the report is Jan. 31, 2027.
To complete this report, you will need to know your total grant allocation and have a copy of ELAP Grantee Report 343: Regular Participants.
Final Local Evaluation Report – June 2027
This report template is for both 21st Century Community Learning Centers (CCLC) and Lottery for Education Afterschool Programs (LEAPs) Grants. An agency with both 21st CCLC and LEAPs funding must complete separate reports for each grant. If an agency has more than one 21st CCLC Grant, separate reports should be completed for each cohort. Updates & Reminders
Reenrolling Attendees: You may have noticed while you’re reenrolling attendees that there are LOTS of them visible, sometimes even in multiples. Don’t be concerned. It is a weird side effect of the merge our developer is currently working on. It will eventually go back to normal, but in the meantime, use the search box to locate your attendees and make sure to select the one most recently enrolled. If you have any concerns, reach out to us at the Help Desk.
Program Performance Outcomes (PPO): Take note: what was once GPRA is now PPO!
Live ELAP Calendar: You can locate all important dates for the program year on SWORPS’s live ELAP calendar on the Important Dates page of our website. If you are part of a Google school district, you can download our calendar and add it to your own work calendar.
Web Resources: As a reminder, we have recently upgraded our website. We are still working on getting all of your resources loaded, so if you can’t find what you need please reach out to the Help Desk so we can get you taken care of.

During that session, SWORPS provided an update and review of the Local Evaluation Report requirements and due dates.
